NOTE: ALL Registrations must accompany a letter from your employer stating that you are employed in that office and that they will supervise your clinical work. Please see "Sample Letter" to the left for a reference. This letter may be emailed, faxed or mailed.
Log-in Information
If you have never created an account through the ADA Registration System, you will need to do so.
Auxiliary Staff - Press the "Create an Account" option and create a username and password.
REGISTER BY FAX OR MAIL - CLICK HERE
- Fax or Email your registration to the information on the bottom of the form.
- Please use one form per person.
- You will not receive a confirmation unless you request one.
Cancellations/Changes: All cancellations or changes to a course registration must be received in writing.
Confirmations: When you register online, you will receive an automatic confirmation via email. If you do not, please check your email spam folder.
